Assessment of Community Subsystems
Purpose
During the NR 538 course, assignments build upon one another as you conduct an assessment of population health within the context of community. While the first assignment involved assessment of the core population, the second assignment addresses assessment of the community subsystems. In the third assignment, analysis of data will be conducted to determine priority needs and propose recommendations to improve population health.
Comprehensive assessment of community subsystems is essential for a holistic assessment of population health. Appraisal of the community subsystems provides valuable insight regarding resources and assets available, as well as gaps in service that may be present. Assessment of subsystems not only informs the appraisal of population health in the context of the community, but also is used to direct interventions to optimize resource utilization.

Students will independently assess the community subsystems where the target population resides. Through this assignment, the student will address the following course learning outcome:
• CO 1: Assess population health and its determinants using quantitative and qualitative methods of data collection. (POs 1, 3, and 5)
ASSIGNMENT OVERVIEW
Identify the community where the target population that was assessed in the Week 3 assignment resides.
Assess the eight community subsystems using the Community as Partner Assessment Model as a framework.
• Seek empirical data and current scholarly sources to assess the subsystems within the community to better understand variables that influence health for the target population.
• Interview key informants to gather qualitative insights regarding community subsystem supports and barriers to health for the target population.
o Remember to follow guidelines for citation of personal communications (such as interviews) using the APA format, current edition.
• Include a minimum of five scholarly sources, current within five years, and at least two key informant interviews to support your findings.
Create a professional PowerPoint presentation to convey your findings. The presentation should be 12– 15slides in length.Follow the assignment criteria and rubric expectations described below to present each of the following:

ASSIGNMENT INSTRUCTIONS
1. Abide by the Chamberlain College of Nursing Academic Integrity Policy.
2. Create the scholarly presentation using Microsoft PowerPoint.
3. Record your narration of the presentation using the “Record Narration” feature in PowerPoint.
4. Cite and reference sources within the presentation using guidelines from the APA Manual (current edition).
5. Slides must professional in appearance with visually balanced text, font size appropriate for audience reading, succinct bulleted points, and visually appealing contrast between background and font utilized. Correct spelling and word usage should be evident on the slide, and if used, graphics and images should be professional and appropriate for the topic.
6. Narration of the presentation should be clear, logical and organized. Timing and flow of information should be appropriately paced to allow the viewer/listener to process information presented. A professional tone should be used, with limited use of filler words.
7. Sources older than five years may not be used without the permission of the class professor.
