Jane Tyler is a 32 year old single mother of 3 children.  She has been living on public assistance for 12 years. Jane has a small apartment above her mother in a less desirable part of town. Since her youngest began school, jane has applied for a grant which will allow her to train to become an information technology assistant so she can make a living.  Receiving this grant was a boost to her self esteem and sees it as an exit from her life situation.  She has nagging anxiety about severe shortness of breath and fatigue she experiences regularly.  She goes to the clinic to have a check up before beginning her education. 

Jane is diagnosed with a chronic lung disease. She has been threatening to stop smoking for several years, but she hasn’t been able to kick the 2 pack a day habit. Her illness is beginning stages.  She may not be eligible for the curative treatment because she is on public assistance. The state legislature presented to the public the opportunity to decide priorities for high cost interventions paid by public funds by sending information to a sample group of physicians.  Three physicians concur that smoking was the direct cause of her illness.  One says there may be co-factors to explore.  Because two of the three  agree on the cause, she is only able to receive basic treatment.  

Her case worker is upset at what he judges to be injustice.  He calls the senator who is sympathetic but knows of no loopholes and is pessimistic about an exception.
